Make an Office Appointment
Call us at (360) 403-0333 on Tuesday through Friday, between 8:00 AM – 5:00 PM to make an appointment. We make every effort to schedule your appointment as quickly as possible. You may leave a message on our voicemail if we are at lunch.
New Patient
Before arriving at your first appointment, please review this list and bring all pertinent
information.
- Your insurance card, driver’s license (or valid picture ID) and co-payment required by
your insurance. Accepted insurance companies.
- A complete and accurate list of your current medications.
- Contact information for your referring doctor so our physicians may send office notes and a letter after your visit.
- Any tests (x-rays, CT scans, MRIs) relevant to your problem.
- Completed New Patient Forms. If you are unable to complete the forms, please arrive 20 minutes before your appointment to give you the time to complete the forms.
Cancel or Reschedule an Appointment
If you need to cancel or reschedule your appointment, please call us at least 24-hours prior to your appointment.
